Nursing Home Violations For The Month Of September

SPRINGFIELD - The Illinois Department of Public Health today announced the following type "A" violations of the Nursing Home Care Act were cited during the month of September.  An "A" violation, which is the most serious licensure violation imposed by the state, pertains to a condition in which there is a substantial probability that death or serious mental or physical harm will result.

A.R.C. of Jacksonville, Ltd., a 93-bed intermediate care facility located at 1320 Tendick, Jacksonville, has been fined $15,000 for failure to prevent sexual and financial exploitation of a resident.  A resident of the facility withdrew all the money from his bank account and temporarily moved in with an A.R.C. staffer.  The facility has requested a hearing on the Department's action. 

Apostolic Christian Timber Ridge, a 98-bed intermediate care facility for the developmentally disabled located at 2125 Veteran Road, Morton, has been fined $50,000 for failure to protect a
resident from neglect.  A resident was found unresponsive and subsequently pronounced dead. 
The facility has requested a hearing on the Department's action.

Avenue Care Center, a 155-bed skilled care facility located at 4505 S. Drexel, Chicago, has been fined $5,000 for failure to sanitize and maintain the floors, walls, ceilings, dining area, laundry room, storage rooms and numerous resident bathrooms.  The facility also is accused of failing to ensure the kitchen was free from a backup of sewage and rodent droppings.  The facility has requested a hearing on the Department's action.

Colonial Plaza, a 70-bed skilled care facility located at 618 W. Goodner, Nashville, has been fined $10,000 for failure to follow its policy to prevent a resident from leaving the facility without staff knowledge.  The facility has requested a hearing on the Department's action. 

Countryside Healthcare Center, a 197-bed skilled and intermediate care facility located at 1635 E. 154 St., Dolton, has been fined $45,000 for failure to administer regularly scheduled insulin and monitor blood glucose levels as ordered by a doctor.  As a result, several residents were hospitalized.  The facility has requested a hearing on the Department's action. 

Genesis House, a 60-bed intermediate care facility for the developmentally disabled located at 350 Sycamore Road, Genoa, has been fined $40,000 for failure to prevent a resident, with a history of exhibiting inappropriate sexual behavior, from attacking two residents.  The facility has requested a hearing on the Department's action.

Illinois Veterans' Home - Anna, a 50-bed skilled care facility located at 792 N. Main St., Anna, has been fined $5,000 for failure to prevent a resident from leaving the facility without staff knowledge.  A staffer saw the resident outside and returned the resident to the facility unharmed.  The facility has requested a hearing on the Department's action. 

Livingston Manor, a 122-bed skilled and intermediate care facility located at 14335 U.S. Highway 66, Pontiac, has been fined $6,000 for failure to supervise a resident who left the facility without staff knowledge, ended up on a two-lane highway and was almost hit by a passing vehicle.  The facility requested a hearing on the Department's action.  After a prehearing, the facility withdrew its hearing request and paid a reduced fine.

Manorcare at Oak Lawn/Kostner, a 144-bed skilled care facility located at 9401 S. Kostner Ave., Oak Lawn, has been fined $25,000 for failure to provide a resident with adequate care.  The resident later was hospitalized and died.  The facility has requested a hearing on the Department's action.

Pekin Manor, a 120-bed skilled care facility located at 1520 E. Camino Drive, Pekin, has been fined $11,000 for failure to follow its abuse prohibition policy after residents' alleged mistreatment by a staffer.  The facility also failed to thoroughly investigate claims of rough treatment.  The facility has requested a hearing on the Department's action. 

Rose Garden Convalescent Center, a 110-bed skilled and intermediate care facility located at 1629 Gardner Lane, Peoria Heights, has been fined $16,000 for failure to prevent the physical abuse of a resident.  A staff member who witnessed the abuse did not intervene to protect the resident.  The facility also failed to follow its abuse prevention policy by allowing the staffer accused of abuse to continue working with residents.  The facility has requested a hearing on the Department's action. 

Sangamon Care Center, a 170-bed skilled and intermediate care facility located at 2800 W. Lawrence, Springfield, has been fined $5,000 for failure to provide adequate supervision of a resident who left the facility without staff knowledge.  The resident was found unharmed outside the facility by a passerby who alerted staff.  The facility has requested a hearing on the Department's action. 

Springfield Terrace, a 65-bed intermediate care facility located at 525 S. Martin Luther King Drive, Springfield, has been fined $5,000 for failure to implement preventive measures to protect residents from abuse by another resident.  The facility has requested a hearing on the Department's action. 

Timber Point Healthcare Center, a 110-bed skilled care facility located at 205 E. Spring St., Camp Point, has been fined $15,000 for failure to report and investigate incidents of suspected abuse against two residents by a staff member.  The facility has requested a hearing on the Department's action. 

Turner Manor, a 35-bed immediate care facility for the developmentally disabled located at 901 Oglesby Road, Harrisburg, has been fined $30,000 for failure to implement policies and procedures against abuse after two residents were physically abused by staff.  The facility has requested a hearing on the Department's action. 

Washington Heights Nursing Home, a 228-bed skilled care facility located at 1010 W. 95 St., Chicago, has been fined $20,000 for failure to consistently monitor and assess the condition of a resident, who later was hospitalized in an intensive care unit.  The facility has requested a hearing on the Department's action. 

Wincrest Nursing Center Corporation, an 82-bed intermediate care facility located at 6326 N. Winthrop Ave., Chicago, has been fined $17,500 for failure to supervise and monitor a resident who was assessed as high risk for leaving the facility.  The resident left the facility without staff knowledge and required hospital care after being found by police.  The facility has requested a hearing on the Department's action.
